For your first visit, plan to arrive 10 minutes early to provide information about your medical history, including current medication and dosage. If you have dental insurance coverage, please provide us with accurate information. Our office is happy to prepare and submit the necessary forms for these benefits. Should any of your medical, address or insurance information change, please notify us so records can be updated.
